A Heavy Heart

Vip patient

Vip patient

Jun 04, 2026

Three weeks after returning to work, Haru finally stepped back into an operating room.
Not as lead surgeon. Observation first.
Smaller procedures. Lower-risk cases. Carefully monitored steps back into familiarity.
At first, even standing beneath the surgical lights again had made something cold crawl down his spine. The smell of sterilization solution still carried echoes of that failed surgery somewhere in the back of his mind. But muscle memory was a stubborn thing. His hands still remembered where instruments belonged. Eyes still tracked monitors automatically. His body still moved instinctively toward precision even when his mind hesitated.
Little by little, the operating room stopped feeling like a graveyard again.
Which was exactly why the assignment waiting on his desk that Thursday morning made his stomach immediately tighten.
Haru stared at the patient file in silence.

VIP Patient Consultation
Vin Laurent.

Even the name was familiar. Practically everyone in the country knew who Vin Laurent was. Young billionaire. Technology investor. Endless business articles. Magazine covers. The kind of wealthy man whose face appeared beside words like visionary and empire.
And according to the chart in front of him, (Severe cardiac valve complication. Surgical intervention recommended.)
Haru frowned slightly as he flipped through the file. Then frowned harder.

Primary surgeon request:
Dr. Haru Sato.

His eyes stopped there.
Surely that had to be a mistake. A knock sounded against the partially open office door before Haru could think further. Dr. Nakamura stepped inside holding a tablet. “You saw the assignment.”
Haru looked up immediately. “…Why me?”
Nakamura raised an eyebrow. “That’s usually how surgery assignments work.”
“You know what I mean.”
The older surgeon studied him for a moment before answering. “He requested you personally.”
That made even less sense.
Haru stared back down at the file. “He could afford literally any surgeon in the world.”
“He can.”
“And after my last major case—”
Nakamura interrupted calmly. “He reviewed your surgical history himself.”
Haru blinked. “…What?”
“He specifically requested the surgeon whose complication rates showed aggressive intervention tendencies during high-risk procedures.”
Now Haru looked genuinely confused. “That’s an insane reason to choose someone.”
“Possibly,” Nakamura agreed.
Then, after a brief pause...
“Which means you’ll probably enjoy talking to him.”
Before Haru could respond to that deeply questionable statement, Nakamura continued. “Consultation room seven. Thirty minutes.”
Then he left. Leaving Haru alone with the increasingly unsettling patient file.

Consultation room seven overlooked the city skyline through wide glass windows washed pale by afternoon clouds.
Haru paused briefly outside the door before entering.
The man sitting inside barely looked up at first. Vin Laurent sat near the window in an expensive charcoal coat with a tablet balanced loosely in one hand. Early thirties, maybe. Sharp features softened slightly by exhaustion beneath the eyes.
Not surrounded by assistants.  Just… calm. Which somehow felt more unsettling.
Vin glanced up once Haru entered. Their eyes met briefly.
Then Vin smiled faintly. “Dr. Sato.”
Haru nodded politely. “Mr. Laurent.”
“Please,” Vin said, setting the tablet aside. “Vin is less exhausting.”
Haru remained standing for a moment before finally sitting across from him.
The silence stretched briefly.
Then Haru opened the file carefully. “I reviewed your scans this morning.”
“And?”
“The valve damage is progressing faster than expected.”
Vin nodded once. “That matches what the other specialists said.”
Other specialists.
Haru folded his hands together carefully. “There are still treatment options available, but surgery is the strongest recommendation.”
“And you disagree?”
“No,” Haru answered immediately. “I disagree with your surgeon selection.”
That finally earned a more visible reaction from Vin. Not offense but interest.
Haru continued before he could reconsider it. “You’ve consulted internationally. Your resources give you access to surgeons with significantly higher success rates than mine in this particular procedure.”
Vin tilted his head slightly. “You looked up my surgical statistics.”
“You looked up mine first.”
That seemed to amuse him. A faint smile touched the corner of Vin’s mouth before fading again. “I did.”
Haru exhaled slowly. “Then you already know about my recent patient fatality.”
The room quieted instantly after the words left his mouth.
Most people avoided speaking about it directly. Haru no longer had the energy to pretend it didn’t exist.
Vin held his gaze steadily. “I know.”
“And you still requested me.”
“Yes.”
Haru frowned slightly. “…Why?”
For a moment, Vin simply looked out toward the city beyond the windows.
When he finally spoke again, his voice was calm. “Because every surgeon I consulted talked about success rates first.”
Haru stayed silent.
Vin glanced back toward him. “You were the only one whose records showed you kept fighting even after the odds collapsed.”
Something uncomfortable tightened in Haru’s chest. “That surgery still failed.”
“Yes,” Vin agreed easily.
The answer startled him slightly.
Vin leaned back slightly in his chair. “Dr. Sato,” he said, “I built my entire life making decisions knowing some of them would fail spectacularly.”
Haru frowned faintly.
Vin’s expression softened into something quieter. “I am not a product of my circumstances,” he said calmly. “I am a product of my decisions.”
Haru looked down briefly at the patient file resting in his hands. “…You should still choose someone else.”
Vin almost smiled again at that.
“You really don’t know how to advertise yourself.”
“This isn’t marketing.”
“No,” Vin agreed. “It’s fear.”
The word landed with uncomfortable precision.
Haru’s jaw tightened slightly.
Vin continued anyway. “You think if something goes wrong again, it confirms whatever ugly thing you’ve been telling yourself since that last surgery.”
Silence...
Because unfortunately… that was true.
Haru lowered his eyes toward the folder. “You’re making a reckless decision.”
“Probably.”
“You barely know me.”
Vin gave a small shrug. “I know enough.”
“That’s not how surgery works.”
“No,” Vin replied quietly. “But trust works that way sometimes.”
The city lights beyond the windows had begun glowing softly beneath the darkening sky outside.
For several long moments neither of them spoke.
Then Vin said calmly, “If you can't do anything about it, then let it go.”
Haru looked up slightly.
“Don't become a prisoner to things you can't change.”
The words hit harder than Haru expected.
Maybe because they sounded less like advice… and more like something Vin himself had learned painfully.
Haru studied him more carefully then. The exhaustion beneath his calm expression. The subtle tension in his posture. The quiet awareness people developed after spending too much time around mortality.
Suddenly Vin no longer looked like a billionaire sitting in a consultation room.
Just another patient trying to place his life into someone else’s hands without certainty.
Something inside Haru shifted quietly at that realization.
Vin broke the silence first. “So,” he asked lightly, “are you going to keep trying to convince me to fire you?”
Haru’s corner of his mouth twitched faintly.
“…I’m still considering it.”
“Terrible customer service.”
Haru almost laughed.
