It's impossible for us to die and there were no past Deaths, it's always been just the 13 of us. The Death you just saw was Death 1, the Death that was born first. As for the last question, what I mean by "Becoming like Naoto" means that the soul stays connected to their body and becomes a D.E.A.D.
