Also side note, I copied the form off examples I found online but someone pointed out that Eskimo is considered a slur apparently? Anyway if someone can confirm and let me know what the appropriate term is I'll go and fix it but I couldn't find the info.

(pdf) and it used "Alaskan Native" with a place to write in specific "enrolled or principal tribe" (This is only from my own research, I'm white and share no connection with the people this might refer to.)
