For one campaign I played at a local gaming bar and we got to witness perhaps the most beautiful moment in all of DND history.

The greatest "fail" I ever had was that both of our fighter rolled a one short after an other.
We were standing back to back. And it was planed that the first who rolls a 1 smacks the weapon out of the hand of the other, but since we both rolled a 1 the DM went "f**k it" and let us drop our weapons in each others hands.
