In the past more people entered the Underworld then left, so the population was always expanding with only souls who had been there for eons being allowed to leave. There used to be a time limit you had to reach in order to be given access to the door, but Hades eventually stopped reinforcing that rule. For a short time Hades had even outright banned souls from the door, but this too was eventually dropped.
No matter how long it takes every soul will choose The Door to Nowhere.
