Imagination is a powerful thing. I would not be surprised if most of this is just imagined by Cathy. That would kind of fit with the whole madness mechanic.
At least it seems to me like magical creatures do not suffer from it? Huh.

In the country of Distantland, nothing is as it seems. Cathryn, a young human girl, prides herself on her knowledge.
